Position Summary and Purpose: In collaboration with the therapist and inter-disciplinary treatment team, the case manager assists to prioritize assigned patient’s current biopsychosocial needs, as well as needs related to discharge and planning resources. They will document individualized interventions on assigned patient’s Treatment Plans.

Requirements

  • Bachelor’s degree in the human services or related field (required)
  • At least 18 months of experience providing substance use, mental health, or co-occurring treatment services in a behavioral health or related human services setting (required)
  • Basic Life Support (BLS) accredited by the American Heart Association (AHA) (required)
  • Able to complete Crisis Prevention Institute Non-Violent Crisis Intervention Training and perform CPI skills and techniques
  • Basic computer skills required, including charting in the electronic health record, using Human Resources online interface (timekeeping, employee file management), UofL Health Intranet (e.g., policies, incident reports, online learning modules), and email

Nice To Haves

  • Master’s degree in the human services or related field (preferred)
  • Three (3) years’ experience providing substance use, mental health, or co-occurring treatment services in a behavioral health or related human services setting (preferred)
  • Proficiency in Microsoft Office preferred

Responsibilities

  • Identifies patient biopsychosocial needs and connects patient with appropriate community resources
  • Interacts directly with patient and guardians to assist in the discharge planning as per the Treatment Plan goals/interventions
  • Coordinates referrals and discharge appointments and collaborates with other service providers and community resources as appropriate
  • Maintains accurate documents of discharge plans and collaboration with community resources, patients and others involved in patient’s care
  • Maintains an up-to-date resource center of community-based resources for clinical team to utilize for patients/family’s needs
  • Assists in therapeutic milieu to provide structured interventions to provide psycho-educational groups to patients, as needed
  • Acts as a patient advocate by ensuring each individual’s rights are maintained at all times
  • Assists patients with completing patient satisfaction surveys near the end of their treatment
  • Stays informed regarding changing status of all patients at the adult outpatient center
  • In collaboration with the treatment team, assists in providing leadership to maintain a safe and therapeutic environment for patients and staff; utilizes proactive techniques to de-escalate the milieu and/or individuals
  • Participates in development, implementation and utilization of therapeutic milieu activities and interventions
  • Maintains compliance with all company policies, procedures and standards of conduct
  • Complies with HIPAA privacy and security requirements to maintain confidentiality at all times
  • Performs other duties as assigned
