About The Position

The Case Manager provides supportive and direct intensive case management services to the homeless families residing in the Shelter. The Case Manager assesses the client’s needs and jointly develops a collaborative plan that addresses those needs. The Case Manager prepares the client for self-sufficiency and independent living and supports their transition back into the community. The Case Manager engages all clients and conferences with them when issues of noncompliance are identified. The Case Manager organizes case conferences and ensures that their clients are actively involved in their discharge planning.

Requirements

  • A Bachelor’s degree in Social Work, Psychology, or a related field.
  • Bilingual - Spanish

Responsibilities

  • Meet or exceed all quarterly performance targets, such as: Public Assistance and Employment, Facility Length of Stay, and Exit and Housing Placements as set annually by the Department of Homeless Services and The Salvation Army leadership.
  • Abide by the requirements set forth in The Salvation Army Nondisclosure Agreement for Social Services Personnel.
  • Oversee caseload of 25 families.
  • Provide supportive assistance to caseload in efforts to identify and resolve issues that interfere with the client’s ability to find placement in permanent housing. This can include but is not limited to public assistance needs, vocational/employment needs, medical needs, mental health needs, educational needs, children’s/parenting needs, independent living skills, financial planning, etc.
  • Actively engage clients in employment and public assistance outcomes.
  • Maintain accurate and detailed case progress notes for every client meeting and engagement. Enter case notes in the approved computer database of client records.
  • Interface with various social service agencies in the non-profit and citywide sector for the purposes of advocacy and/or education on new referral sources.
  • Arrange meetings for overall assessment and intake of newly assigned residents within 24 hours of admission.
  • Perform all concrete casework requirements and referrals, including assessment and entitlement, in a timely manner.
  • Identify clients in need of external services, contact these social service agencies regarding the client, and help in establishing relationships with other service providers.
  • Complete all required paperwork, i.e. bi-weekly service plans, case management notes, referral sheets and statistical requirements on a timely basis. Input a minimum of 15 contacts per week in the database system.
  • Engage and reinforce clients in discussions about Residence rules and regulations and Department of Homeless Service Code of Conduct and Client Responsibility.
  • Make recommendations for next step referrals and involuntary transfers for noncompliant clients and/or clients that are not making significant progress towards existing the homeless system.
  • Provide follow-up and resolution to all client incident reports and complaint reports as is required.
  • Actively participate in all required client hearings and discharges.
  • Support client’s goal of finding long-term permanent housing as part of the social service team.
  • Participate in weekly housing, intake and case conferencing staff meetings.
  • Actively engage in weekly clinical supervision with Case Manager Supervisor.
  • Be assigned to work one late day per week (12pm – 8pm) and one rotating weekend shift per month.
  • Assume same position at one of our other family shelters operated by the Salvation Army at the discretion of the Director of Shelters or Salvation Army Department Heads, if asked.
  • Undertake other assignments and tasks as may be added at the discretion of the Director of Shelters or Salvation Army Department Heads.
