About The Position

Ochsner Rehabilitation Hospital, a joint venture/partnership/affiliation with Select Medical, is hiring a per diem Case Manager in New Orleans, Louisiana. The inpatient rehabilitation hospital provides care for people facing medical, physical, emotional, and vocational challenges related to brain injuries, spinal cord injuries, neurological disorders, orthopedic issues, amputation, and multiple traumas.

The Case Manager coordinates healthcare decisions using a systematic approach to support treatment plans that improve quality and outcomes. The role coordinates care across the continuum, promotes cost-effective care within the allotted time frame, ensures payment for hospital-based services meets patient-related utilization management criteria, and implements safe and appropriate discharge plans. The Case Manager also assesses patients’ psychosocial needs and provides intervention as part of discharge planning.

The position is per diem/as needed and pays $42.00 per hour. Part-time and per diem employees may qualify for a 401(k) after reaching 1,000 hours within their first anniversary or a subsequent calendar year. Requirements

• Current licensure in a clinical or related discipline, in accordance with state guidelines, or a bachelor's or master's degree in a health or human services discipline.

Nice To Haves

• Previous experience in case management and discharge planning.
• CCM certification.
