The IOC Case Manager is a full-time position responsible for facilitating and participating in a multi-disciplinary treatment team. This role involves regular communication and collaboration within the IOC court system and with the IOC team, including physicians, court staff, judges, public defenders, and other legal teams. The goal is to reduce the risk of rehospitalization and ensure consumer compliance with indicated treatments. The Case Manager will engage consumers in treatment planning and all areas of service delivery, provide direct care both in-home and in-community as part of the multi-disciplinary team, and assist consumers with activities of daily living through modeling, direct assistance, and coaching. Additionally, the role provides direct assistance with obtaining basic needs and offers crisis intervention as necessary.
