As a Case Manager, you'll play a vital role in helping clients achieve meaningful progress by providing rehabilitative services, coordinating care, supporting individualized goals, and building trusted relationships that foster lasting recovery. Case Managers provide psychosocial rehabilitative services as indicated by the client’s care plan to support clients in meeting their individualized goals. Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met.
