About The Position
The Case Manager is responsible for interdisciplinary coordination of care for a designated patient population. The role includes reviewing inpatient admission records to ensure appropriate use of hospital resources and confirm that admissions meet the appropriate level of care. The Case Manager assesses discharge needs and coordinates an appropriate discharge plan, working collaboratively with the interdisciplinary team to support desired financial and quality outcomes. The position also involves maintaining and enhancing payor relationships.
Requirements
- Current BLS certification is required.
- Three to five years of inpatient experience.
- Professional discipline may be occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
Nice To Haves
- Licensure in the state where the hospital resides is preferred.
- Inpatient experience in an acute, IRF, or LTACH setting is preferred.
