The Case Manager will build relationships with patients at the RCC experiencing homelessness, and assist with basic needs assessments, housing search, access to public benefits and other resources as appropriate. The role involves providing thorough screening for CSP-HI eligibility, engaging with new clients, and assisting clients with physical and mental health needs. This includes securing health insurance, addressing transportation, providing medication reminders and health education, connecting clients with support groups, providing personal hygiene products, and coordinating income generation, housing resources, and food access. The Case Manager will collaborate with the LCHC medical team for various services, attend team meetings, and communicate concerns as defined in RCC Protocols. They will also register clients through the Homeless Management Information System (HMIS) for coordinated entry access, partner with the Direct Care Supervisor for intake, and coordinate/facilitate enrichment activities and psychoeducational groups. Conducting housing assessments, assisting with housing applications, mitigating barriers, and gathering documentation are key responsibilities. The role also involves assessing for crises, engaging in discharge planning, collaborating with community service providers, and advocating for clients. A Trauma Informed Care model is to be utilized, ensuring clients are treated with dignity and respect, and services comply with funding sources. Thorough and timely documentation and communication are essential.
