About The Position

Westchester Jewish Community Services (WJCS) is an award-winning, nonprofit, nonsectarian human services organization founded in 1943. Today, it is the largest provider of licensed outpatient community behavioral health services and one of the largest human service agencies in Westchester County. WJCS delivers innovative programs and services and compassionate care to more than 20,000 people of all ages and backgrounds throughout the county. We are excited to announce an opportunity for a full-time Case Manager to join a groundbreaking new program dedicated to addressing health-related social needs and improving overall outcomes for Medicaid recipients. This innovative initiative connects individuals to essential community resources that support their overall well-being and health outcomes. In this role, you will be responsible for conducting comprehensive intake and eligibility assessments, facilitating service navigation, and ensuring participants receive timely and effective care coordination. The salary for this full-time position is $55,000.

Requirements

  • Bachelor’s degree in Social Work or related field required.
  • Minimum of 1 year of experience in case management, social services, or healthcare required.
  • Strong interpersonal, communication, and organizational skills.
  • Ability to work independently and collaboratively in a team-based environment.
  • Compassionate, strengths-based approach to working with diverse and underserved populations.
  • Ability to document and manage information across multiple systems.
  • Basic computer proficiency required.
  • Ability to travel throughout Westchester County, as needed.
  • Valid driver’s license required.

Nice To Haves

  • Experience working with disenfranchised individuals facing complex barriers preferred.
  • Bilingual English/Spanish strongly preferred.
  • Ability to work flexible schedule, including weekends, as needed.

Responsibilities

  • Conduct intake interviews, screening, and comprehensive assessments for program eligibility and service needs.
  • Provide person-centered case management and connect individuals to appropriate community resources.
  • Support service navigation and assist clients in accessing needed and available community resources.
  • Maintains accurate and timely documentation in Unite Us and the SCN tracking system.
  • Participates in regular case management team meetings to discuss progress, outcomes, and/or new or existing barriers to meeting program goals.
  • Collaborative effectively with the CCBHC and SCN team.
  • Maintain compliance with agency policies, procedures, and professional ethical standards.
  • Completes daily documentation on services provided.
  • Meets and exceeds service deliverables.
