About The Position

The Doe Fund is seeking a Case Manager for the Webster Green Residence, a permanent supportive housing program for homeless individuals living with serious mental illness and/or a history of substance use. This program offers long-term residential care and 24-hour support services in a community environment designed to maximize resident independence while ensuring their physical and emotional security. The Case Manager will support residents referred by HASA, providing comprehensive case management and counseling tailored to individuals with co-occurring disorders and/or trauma. The Doe Fund's mission is to break the cycles of homelessness, addiction, and recidivism through its Ready, Willing & Able program, which offers paid transitional work, housing, training, and supportive services. The organization is also a major developer of affordable and supportive housing in New York City.

Requirements

  • A Bachelor’s degree in Psychology, Mental Health Counseling, Social Work, or a relevant field.
  • At least 3 years of experience working with homeless individuals diagnosed with serious mental illness and/or substance abuse disorder and/or in trauma settings
  • Expertise in Evidence-Based Practice, including Critical Time Intervention, Motivational Interviewing, and Harm Reduction techniques
  • Excellent clinical and case management skills, including knowledge of benefits in the NYC Metro area
  • Sensitivity to community issues and needs
  • Outstanding interpersonal and communication skills, both oral and written
  • Strong ethical standards, confidentiality, and personal integrity
  • Computer proficiency (i.e., email, Microsoft Word, Excel, and PowerPoint)

  • Master’s degree preferred.

Responsibilities

  • Maintain a caseload of individuals diagnosed with serious mental illness and/or substance use disorder and provide supportive case management services to them
  • Develop and facilitate therapeutic groups for residents
  • Maintain resident records in compliance with agency and funding standards
  • Conduct and review assessments
  • Develop and implement individual Service Plans and assist residents with achieving their goals geared towards self-sufficiency
  • Monitor and ensure residents are linked to appropriate treatment and services
  • Provide counseling and crisis intervention as necessary
  • Assist residents in securing and maintaining benefits
  • Maintain a liaison with community services, including hospitals, substance abuse treatment programs, HRA, and mental health programs
  • Monitor and assist residents with rent payments and budgeting
  • Conduct regular unit inspections
  • Educate residents about approaches to mental health and substance use
  • Compile resources on mental health information and treatment strategies
  • Attend ongoing training to enhance the services provided to the residents
  • Assist with the completion of reports as needed
  • Other duties as assigned
