About The Position

We currently seek a full-time Case Manager to join our team at Osborne Association's Marcus Garvey Supportive Housing Developments. The Case Manager will specifically oversee medical cases and documenting complex co-occurring conditions. Marcus Garvey 1 and Marcus Garvey 2 are located in the Brownsville neighborhood of Brooklyn and have a combined 140 units of supportive housing for adults 50 years of age or older who are reentering the community from incarceration and who would otherwise be homeless or unsuitably housed. The services provided by Osborne will be designed to address all the aspects of life that a formerly incarcerated person needs to rebuild upon reentry to the community. The Case Manager will be responsible for (with particular emphasis on those with complex medical needs); design and supervision of group counseling, conducting case conferences, treatment plan reviews, and maintenance of established QA procedures; and providing trauma counseling support when needed. This position reports on-site to our office in Marcus Garvey 1 &2, located in Brooklyn, NY.

Requirements

  • Bachelor's degree in criminal justice, social work, psychology, or related field preferred with at least 3-5 year of case management experience within a social service environment or relevant work experience
  • In lieu of degree, High School Diploma or equivalent with a minimum of five years of progressive case management experience working with the criminal justice population
  • 1-2 years of experience of trauma counseling and treatment plans for medically complex cases
  • Experience working with older adults with medical needs
  • Engaging personality and detailed oriented skills are essential
  • Strong oral and written communication, time management, and organizational skills are necessary
  • Understanding and some knowledge of working with the elder population with medical needs
  • Familiarity with community resources
  • Ability to travel locally as needed
  • Ability to solve problems, make decisions, resolve conflicts and actively listen
  • Access skills to deal calmly in crisis situations
  • Proven ability to work collaboratively well with diverse groups
  • Must be able to handle multiple tasks effectively under pressure

Responsibilities

  • Provide crisis intervention and stabilization as needed in the program
  • Manage a caseload for a diverse client population, overseeing medical cases and documenting complex co-occurring conditions
  • Coordinate client services with other social services, mental health, medical, employment, educational, and other providers
  • Provide direct client services, including outreach and screenings via videoconference or in person, intake, engagement, assessment, service plan development, referral, advocacy, counseling, exit summaries, and follow-up
  • Monitor and document client progress toward service plan goals
  • Document all interactions with, or on behalf of, clients, including emergency services and other relevant healthcare referrals
  • Maintain familiarity with program resources available on- and off-site
  • Conduct case conferences with clients, service providers, and attorneys as required
  • Attend court dates with clients as needed
  • Write brief update letters to the Court to address client progress
  • Report to and meet with supervisor on a bi-monthly basis or as needed for supervision
  • Ensure adherence to all required regulations and OTDA policies and procedures
  • In-person attendance is required to collaborate with co-workers
  • Perform other duties as assigned
