About The Position

Avenues Recovery Center is a nationwide network of drug and alcohol rehab centers with eighteen locations across seven states, known for its evidence-based clinical curriculum and highly individualized care. The organization offers programs including detox, residential, PHP, IOP, and outpatient services, aiming to transform lives in the recovery community. The Case Manager will advocate for clients, address external barriers to treatment, initiate and provide ongoing case management, oversee client accounts to ensure active insurance, and arrange necessary post-discharge support such as housing, financial aid, legal aid, and vocational training. They will also facilitate comprehensive discharge planning, including continuing care and sober support. The role is based at Avenues Recovery Center at Lake Ariel, a 40-bed residential facility in the Poconos mountains, providing a supportive environment for client recovery.

Requirements

If in recovery, a minimum one-year period of sustained sobriety is required
Excellent communication, multitasking, and time management skills
Strong problem-solving and critical thinking skills
Warm, empathetic, respectable personality

Nice To Haves

High School diploma or GED
Min. 1-year experience in treatment industry

Responsibilities

Advocate for client, addressing and resolving any external barriers to treatment
Initiate case management for each client upon intake, addressing any pressing needs
Oversee client accounts, ensuring insurance is active throughout duration of treatment
Provide ongoing case management throughout the duration of treatment
Arrange housing, financial aid, legal aid, vocational training etc. as necessary for client post - discharge
Facilitate comprehensive discharge planning (continuing care, housing, sober support etc.)
