Gilead Community Services is seeking a compassionate, person-centered professional to join our WISE (Working for Integration, Support, and Empowerment) team. The WISE program empowers individuals managing mental health challenges by providing essential supports that help them live safely, independently, and successfully in their communities. As a Clinical Case Manager, you will serve as a dedicated advocate, modeling wellness strategies to help individuals successfully transition to or maintain community-based housing. This position is scheduled Monday through Friday, 8:00 AM to 4:00 PM (40 hours per week), with occasional weekend flexibility as needed. In this role, you will work side-by-side with adults living with serious mental illness and co-occurring substance use disorders to develop and strengthen critical daily living skills, including cooking, home organization, medication management, budgeting, and personal hygiene. Support extends beyond the home through assistance with medical and behavioral health appointments, shopping, accessing local resources, building social connections, and developing healthy coping mechanisms. Clinical Case Managers also play an important role in coordinating services, monitoring progress, and maintaining timely and accurate documentation, including assessments, treatment plans, progress notes, and other required records.
