About The Position

Coordinate preconceptional and interconception services for prenatal, non-pregnant high-risk women and their partners. This role involves assessing risk, developing risk reduction goals, linking participants to services, and ensuring the development of a comprehensive Life Course Plan. The Case Manager will also link male partners to group activities and individual services, assist with and ensure participation in scheduled Life course group activities, and provide services in the participant's home or current living location. Additionally, the role includes providing social and mental health linkages and support, following up with all providers involved with the family, providing feedback to agencies, and participating in case conferences as required. All documentation must be completed and maintained, ensuring confidentiality.

Requirements

  • Two years’ experience as a case manager.
  • Bachelor degree in the field of maternal child health or social science preferred.
  • Will substitute 5 years professional experience in direct service, social services, maternal and child health or related field.
  • Experience working with diverse communities, organizations and agencies preferred.
  • Knowledge of local health and human service providers
  • Ability to work flexible traditional and non-traditional hours
  • Ability to be a self-starter and work independently as necessary
  • Good communication oral and written
  • Maintain professional boundaries
  • Ability to work independently and to carry out assignments to completion within parameters of instructions given prescribed routines, and standard accepted practices.
  • Must be able to communicate effectively and be a team player at the Coalition.
  • Must be able to commit to the Coalition vision and be passionate about its mission.
  • Requires reliable transportation as travel is required, valid driver’s license, good driving record and automobile insurance.

Responsibilities

  • Coordinate prenatal, pre- and interconception services for high-risk women.
  • Utilize selected standardized curriculum, intervention, and document activities.
  • Assess risk, develop risk reduction goals/activities, and link participant to services.
  • Ensure that a comprehensive Life Course Plan is developed for each participant.
  • Link male partners of enrolled women to group activities and individual services.
  • Assist with and ensure participation in scheduled the Life course group activities.
  • Provide services in participant’s home, or current living location.
  • Provide social and mental health linkages and support.
  • Follow up with all providers involved with the family.
  • Provide feedback to all agencies involved.
  • Participant in case conference as required.
  • Complete and maintain all documentation, ensuring confidentiality.
  • All other duties as assigned by the Team Lead or Magnolia Project Director.
