As a Prevention Intensive Case Manager, you will provide six to nine months of intensive services to families whose children are either at risk of being removed from their homes or are already in placement. The goals of the program are to prevent out-of-home placement for children with multiple stressors, provide a safe environment to return children already in placement, and prevent future out-of-home placement. You will report to the Program Coordinator and work within a treatment team, including a Clinician and/or a Community Support Worker depending upon the needs of each family. You will work a combination of on-site and remote with travel to clients' homes required.
