Our organization offers 50+ programs that address mental health, the unhoused, early care and education, those impacted by the justice system, transitional age youth, as well as aging adults. Our programs combine the latest scientific research with cultural sensitivity and a deep commitment to supporting and reflecting the communities we serve. The Adult Protective Case Management Program, under contract with Alameda County Adult Protective Services, provides time-limited case management services with the goal of assisting functionally impaired seniors and adults with disabilities obtain services that promote and maintain optimal functioning, as identified by APS. These services are provided within a system of support within the agency and with close collaboration with other support systems, such as In-Home Supportive Services, Meals on Wheels, housing, and other services. A specialization of this program is the additional expertise within the agency of mental health assessment and services, with a goal of providing warm linkages for clients to mental health services, if needed.
