About The Position

The Adult Protective Case Management Program, under contract with Alameda County Adult Protective Services, provides time-limited case management services with the goal of assisting functionally impaired seniors and adults with disabilities obtain services that promote and maintain optimal functioning, as identified by APS. These services are provided within a system of support within the agency and with close collaboration with other support systems, such as In-Home Supportive Services, Meals on Wheels, housing, and other services. A specialization of this program is the additional expertise within the agency of mental health assessment and services, with a goal of providing warm linkages for clients to mental health services, if needed.

Requirements

  • A minimum of 2 years of experience working with older adults
  • Strong collaborative skills with team members and community partners
  • Solution-focused, fast-paced, person-centered
  • Knowledge of Alameda County community resources
  • Knowledge and/or experience work with Adult Protective Services (APS)
  • Knowledge of human behavior and performance
  • Social Perceptiveness Skills; Awareness of others' reactions and understanding why they react as they do
  • Ability to communicate information and ideas in speaking and written form so that others can clearly understand
  • Anticipatory problem-solving skills
  • Ability to be a team player with a welcoming attitude

Responsibilities

  • Intake, assessment, completing the care plan as provided by APS or updating it, coordinating among service providers, following up and monitoring, reassessment, and discharge and transitioning care within six months.
  • Uses the agency's electronic health record.
  • Participates in a supervisory collaborative and meets regularly with the supervisors and other case managers in Alameda County for case review and consultation, as well as participating in relevant training and collaborative efforts within the agency.
  • Advocating with outside agencies on behalf of clients to secure benefits, housing and resources.
  • Maintaining a professional working relationship with providers and other support systems.
  • Works with other Felton clinical case managers to allow for increased access to mental health services or other services needed for the APCM clients.
