Under the supervision of the Case Management Program Manager, the Case Manager - Clinical will be responsible for providing ongoing engagement, assessment, intervention, and evaluation for the youth on their individual case load. This includes youth intakes, assessments, crisis phone counseling, client data collection, documentation, file maintenance, and follow-up care. In addition, the Case Manager will assist with general upkeep of the case management program, including regular program shift coverage, milieu management, crisis intervention, and ensuring that the program is facilitated in a manner that meets Urban Peak’s mission. Case Managers will work with youth ages 12-24 who are enrolled in Urban Peak’s programming who are experiencing homelessness. The acuity of clients varies, but this role will specialize in working with higher acuity clients to achieve goals including improved mental health and substance misuse issues. Weekly clinical case management sessions are conducted with each youth to review progress towards objectives and to provide support and accountability. This position will ensure that interactions, interventions, and environments at Urban Peak are trauma-informed and emphasize the physical and emotional safety of youth and staff.
