About The Position

St. Patrick Center provides opportunities for self-sufficiency and dignity to people who are homeless or at risk of becoming homeless. Individuals and families build permanent, positive change in their lives through safe and affordable housing, sound mental and physical health, and employment and financial stability. Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St. Louis region through Housing First, an evidence-based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals. St. Patrick Center, a ministry of Catholic Charities of the Archdiocese of St. Louis, is seeking a Case Manager to be responsible for overseeing the daily care and support of clients at Rosati Place, specifically within the Semi-Independent (SEMI) program. This role provides individualized counseling, treatment coordination, rehabilitation support, advocacy, and comprehensive case management services to clients who have co-occurring mental health and substance use disorders. Case Managers work closely with clients to promote stability, independence, and overall well-being while ensuring access to appropriate resources and services. This position may include both full-time and weekend schedules, requiring flexibility to meet the ongoing needs of the program and its clients.

Requirements

  • MA degree in Social Work or Counseling and/or QMHP
  • Experience in counseling and case management
  • Experience in crisis intervention and working with difficult clients
  • Knowledge/understanding of area/community resources, to provide appropriate referrals
  • Knowledge of Federal, State, and Agency policies of HIPPA confidentiality.
  • Driver license required (MO residents require a Class E).
  • Reliable vehicle and current and valid auto insurance required.
  • Delivers clear written documentation and conveys information verbally with accuracy and professionalism.
  • Applies ethical standards consistently; protects client information; exercises sound judgment.
  • Uses established intervention techniques; identifies risk; engages appropriate supports during crises.
  • Adapts approaches to diverse backgrounds; demonstrates awareness of cultural factors in service delivery.
  • Contributes effectively in team settings; communicates needs; supports shared goals.
  • Prioritizes tasks, meets deadlines, and maintains productivity with minimal supervision.
  • Uses office and record‑keeping systems accurately; maintains error‑free documentation.
  • Produces precise work, follows procedures, and maintains consistent reliability in fast‑paced environments.

Responsibilities

  • Develop and maintain a positive and therapeutic relationship with each client.
  • Be familiar with each client’s diagnosis, medications and history in order to assist in the implementation of each client’s treatment goals.
  • Keep client files/paperwork organized and continuously update client files and internal files, to reflect program and audit needs.
  • Conduct comprehensive, ongoing assessments of all client’s mental illness, substance use, physical health, housing status, and other relative needs in accordance with housing stability.
  • Understand different theories and modalities within client engagement, including Housing First knowledge.
  • Provide direct clinical services to all clients on an individual and group basis in the office and community settings (as needed), making appropriate changes to treatment plans to ensure the client’s strengths, needs, and resources.
  • Assist in creating a comfortable atmosphere for clients.
  • Assist in overall continued program development within Rosati and individual programming.
  • Treat each client with respect, keeping good professional boundaries with residents and staff.
  • Report behavioral/physical changes, problems, or concerns regarding clients to appropriate individuals and respond/intervene as needed in a therapeutic manner.
  • Complete daily case notes and individuals with all clients daily.
  • Complete group education and counseling with client caseload.
  • Maintain all client files and notes per auditing oversight.
  • Update all client goals, outcomes, and treatment plans into the CaseWorthy system files.
  • Communicate regularly with supervisor for performance and feedback, as well as special assignments.
  • Peoples HC daily note requirements.
  • Monitor the MAR for accuracy in coordination with Rosati Nurse, as applicable.
  • Available for on call crisis support/coordination, as needed.
  • Other duties/tasks as assigned.
