About The Position

IPMG is seeking a Case Manager for Howard County. This hybrid position requires travel to community-based meetings multiple times per week and involves working remotely from a home office setting. IPMG is 100% Employee Owned through an Employee Stock Ownership Plan (ESOP), allowing employees to share in the company's growth. The role involves providing information and education on the Medicaid waiver process, creating Individual support plans, helping Individuals access necessary services, and facilitating community-based team meetings.

Requirements

  • A valid driver's license, car insurance, and reliable transportation
  • A Bachelor’s Degree in Human Services
  • A minimum of one year of experience working with Individuals with intellectual and developmental disabilities
  • Designated home office setting with computer, internet service, and smart phone

Responsibilities

  • Provide information and education to help Individuals understand the Medicaid waiver process
  • Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
  • Help Individuals access services and supports they need to meet their goals
  • Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
