About The Position

The Case Manager serves in an entry-level, case management–focused role responsible for providing non-clinical support services to individuals with behavioral health needs. This position plays a critical role in client engagement, care coordination, and service access by assisting individuals in navigating healthcare, social services, and community-based resources. Working under the supervision of licensed clinical staff, the Case Manager supports continuity of care, promotes client stability, and helps address social determinants of health that impact overall well-being.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Human Services, or a related field.
  • Valid driver’s license and reliable transportation required for community-based services and home visits.

Nice To Haves

  • Experience in case management, social services, or behavioral health preferred
  • Experience working with vulnerable populations including pregnant women or families preferred
  • CPR/First Aid (preferred)
  • CPI (Crisis Prevention Intervention) or equivalent de-escalation training (preferred)

Responsibilities

  • Conduct outreach and engagement activities in community, home, and clinic-based settings
  • Conduct needs assessments and crisis intervention services (e.g., IM+CANS, LOCUS)
  • Establish and maintain supportive, professional relationships with clients
  • Assist with client intake, orientation, and ongoing engagement efforts to reduce barriers to care
  • Support care coordination efforts, including appointment scheduling, reminders, and follow-up
  • Plan and implement individual and group therapeutic activities.
  • Collaborate with interdisciplinary teams to ensure continuity of care and service alignment
  • Assist in developing and implementing individualized service plans under supervision
  • Monitor client progress and communicate updates to clinical staff
  • Connect clients to community resources including housing, transportation, employment, food assistance, and healthcare services
  • Assist clients in completing applications and accessing public benefits or community programs
  • Advocate on behalf of clients to reduce barriers and improve access to services
  • Provide non-clinical support during crisis situations, including de-escalation and coordination with clinical staff
  • Follow established protocols to ensure client safety and appropriate escalation when needed
  • Maintain accurate and timely documentation, including service logs, outreach activities, and client interactions
  • Ensure compliance with organizational policies, confidentiality standards, and regulatory requirements (e.g., HIPAA)
  • Work closely with clinicians, counselors, and case managers to support service delivery
  • Participate in team meetings, case reviews, and supervision sessions
  • Support administrative and programmatic needs as assigned
