About The Position

Pilsen Wellness Center is seeking a dedicated and compassionate Case Manager to participate in the implementation of the individual’s service plan, specifically designed to increase an individual’s skills in independence in daily living, economic self-sufficiency and community integration.

Requirements

  • High School Diploma or GED
  • Active Drivers License
  • 3-5 years of administrative and clerical assistance experience
  • Cultural humility and the ability to build trust with diverse populations
  • Strong documentation and collaboration skills

Nice To Haves

  • Bilingual (English/Spanish) preferred

Responsibilities

  • Assists the client in achieving independence (e.g., housing, finances)
  • Assists with supported employment/education as needed
  • Maintain timely and accurate clinical documentation
  • Helps participants to access the services specified in their service plans
  • Adhere to ethical and confidentiality standards
