About The Position

The Case Manager is responsible for performing a wide variety of patient care activities as directed by the Clinical Director. These duties will be to have the responsibility for maintaining communication with the referral source and other services provided as needed. Assist in coordinating discharge planning. Delivers support and addresses clients’ needs with clinical knowledge and interpersonal communication skills.

Requirements

  • Bachelor's degree in behavioral health field required
  • 1 year of experience directly related to the duties and responsibilities specified required.
  • Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience.
  • Minimum of beginner skills in Microsoft Word, Excel, and PowerPoint.
  • Intermediate skills are required with Microsoft Outlook.
  • Must be able to effectively maneuver within an office scheduling system.
  • Must be able to efficiently learn and effectively work within an electronic medical records system.

Nice To Haves

  • Master’s Degree preferred

Responsibilities

  • Facilitate referral and patient access to community resources, including housing, food, clothing, educational supports, vocational opportunities or services, life skills, mental health services, needed medical support and social/community supports.
  • Facilitate discharge planning to ensure continuity of care and ongoing support with providers post discharge from EDCare.
  • Responsible for ensuring all components of the discharge plan are in place to facilitate timely release, according to policy.
  • Communicates pertinent clinical information to outside providers or other individuals as helpful for coordination of care and resource development.
  • Assist treatment team with completing patient’s STD, FMLA, and miscellaneous documentation.
  • Review assessments from referring facilities and note problems/issues that require further attention.
  • Participate effectively in multidisciplinary treatment plan reviews, demonstrating adequate preparation and understanding of patient issues, progress, areas of resistance, positive and negative strategies, etc.
  • Remains knowledgeable of community health care, educational supports, vocational services, and support services.
  • Effective in finding new and existing resources needed for continued patient care.
  • Monitor, evaluate and record client progress and adjust plans and services as needed in collaboration with other team members.
  • Strong interpersonal and communication skills with the ability to build rapport quickly, facilitating positive relationships across a wide range of constituencies, including patients, families, multidisciplinary teammates, and school-based personnel.
  • Works effectively with a wide range of individuals and systems in a diverse community.
  • Possess exceptional work ethic and ability to provide quality assistance while multitasking.
  • Displays strengths in communication, written and verbal, and prioritization and delegation.
  • Utilize an EMR to document patient interactions and document all patient encounters appropriately and timely.
  • Prepare and fulfill records requests, after release of information has been approved by compliance.
  • Provide new patient orientation.
  • Provide coverage for primary therapists when OOO to include POD, group facilitation, and individual sessions.
