About The Position

The Case Manager position will develop a comprehensive support program for youth across Threshold programs. The goal of the program is to build protective factors and mitigate risk factors for youth through building trust, self-esteem, a sense of connection to the community, and by reducing social isolation. The Case Manager leads the coordination of case plans and works with other program staff to ensure consistency and follow-though. This position is a temporary full-time role (37.5 hours per week), with hours of work from Monday to Friday, 9:30 a.m. to 5:30 p.m. The position will run from July 2026 through September 21, 2026. The wage range for this position is $28.00 - $32.47 per hour and is dependent on experience and related skills. The successful candidate can expect to enter the organization no higher than the midpoint of the compensation range.

Requirements

  • Possess a relevant degree such as Social Work or Child & Youth Care.
  • Minimum of 2-3 years experience working with vulnerable populations and at-risk youth.
  • Advanced knowledge of case management principles.
  • Ability to keep excellent client records, both written and electronically.
  • ASIST and NVCI training.
  • Strong working knowledge of computer systems and programs, social media and technology that youth access.
  • Criminal Record Review Check completed by and satisfactory to Threshold.
  • Sign Code of Conduct and Confidentiality Agreement.
  • Requires daily travel to accommodate engagement with youth at various locations and to support other Programs at Threshold as needed.
  • Ability to work independently and as a member of a wraparound care team.
  • Criminal record check prior to employment, and subject to regular review.
  • Up-to-date Emergency 1st Aid certificate.
  • Valid driver’s license and access to a reliable vehicle.
  • Up-to-date membership in relevant professional association or societies.
