About The Position

As a Case Worker, you will provide critical clinical and practical support to older adults and their families through comprehensive in-home and in-office assessments. Working under the guidance of Master’s-level staff, you will develop tailored care plans, coordinate life-essential services, manage local programming, and supervise support staff to help seniors preserve their independence and age safely in place. Requirements

  • Graduate of an accredited college or university with a BSW, BA, or BS degree (or equivalent).
  • Computer literacy with the ability to manage electronic records; strong writing and data organization skills.
  • Proficiency in English required.
  • Ability to travel to clients’ homes using mass transit and navigate physical barriers (stairs, etc.) in various residences.
  • A deep understanding and appreciation of how race, religion, ethnicity, sexual orientation, and individual values shape service delivery for diverse families.

Nice To Haves

  • knowledge of Yiddish, Russian, Spanish, or Patois is highly helpful.

Responsibilities

  • Conduct comprehensive in-home and in-office assessments of clients' social and emotional needs using standardized tools (e.g., PSA, EISEP-PATH).
  • Develop and execute written case plans; provide individual/family counseling and guidance to preserve client autonomy.
  • Screen clients for entitlement eligibility, complete necessary applications, and fiercely advocate to secure benefits (Medicare, Medicaid, public assistance).
  • Arrange critical direct services including homecare, transportation, meals on wheels, legal aid, and emergency psychiatric or medical care.
  • Evaluate clients' capacity to manage their own affairs; protect seniors from financial exploitation and assist with guardianship or emergency relief fund disbursements.
  • Organize daily community activities and monthly events; facilitate group counseling, community education, and caregiver support groups.
  • Supervise community aides, Title V workers, homemakers, home health aides, interns, and volunteers.
  • Maintain precise electronic case records, reports, and statistics; represent and interpret JASA’s mission at community conferences and meetings.
