About The Position

The Case Manager (CM) is responsible for assisting the Clinical team in coordinating, implementing, and executing various patient-related affairs to support patients during their treatment. The CM is responsible for recording and monitoring patient communications, activities, and interactions within the facility. Additionally, the CM assists in overseeing patients' daily schedules in accordance with facility policies, procedures, and State and Federal Regulations.

Requirements

  • At least 2 years of working with people with behavioral health needs (trauma, mental illness and/or substance abuse)
  • Experience working on a multidisciplinary team
  • Tenacity and ability to think out of the box and work creatively to engage participants
  • Knowledge of the criminal justice system and community resources
  • Computer literate. Proficient in Microsoft Office applications
  • Excellent verbal and written communication skills
  • Ability to work as a team member and have management, communication, organizational and interpersonal skills
  • Ability to work under stressful conditions and be flexible in relation to department needs
  • Knowledge of Joint Commission standards within the Department
  • Demonstrates Proficiency in Verbal and written Communication Skills
  • Knowledge of State and Federal Statutes Regarding Patient Confidentiality Laws
  • Knowledge of Drug Free Workplace Policies
  • Knowledge of Workplace Violence
  • Knowledge of current technology
  • Knowledge of Substance Use Disorder
  • Familiar with community resources

Responsibilities

  • Assist with the supervision of patients' in their daily activities.
  • Conduct Case Management Initial Assessments.
  • Write Daily progress notes.
  • Engage in Daily Department and Cross Departmental communication/Treatment team meetings.
  • Perform Daily documentation and facilitation of assignment notes.
  • Run, document, and facilitate daily activities and their associated notes.
  • Run, document, and facilitate daily groups and their associated notes.
  • Work with therapists to ensure Discharge planning and Instructions are completed throughout the patient’s treatment and are clear and complete.
  • Assist with and complete phone calls with patients and any communication with outside providers.
  • Conduct assessments of the client's legal, vocational/educational, and basic life needs and facilitate a bridge between the client and community resources to resolve the client's needs or issues.
  • Assist in completing Coordination of Benefits documents.
  • Facilitate, record, and complete Family Pre-assessments.
  • Assist with and record possible discharges, transfers, and AMAs (Leaving Against Medical Advice).
  • Ensure the patient’s follow clinical schedule.
  • Facilitate patient tours of the treatment facilities.
  • Redirect and report patients if rules are broken.
  • Seek corrective criticism and evaluate suggestions objectively.
  • Maintain acceptable overall attendance.
  • Promote a favorable/positive work atmosphere.
  • Attend in-services and educational training as necessary and assigned.
  • Seek out learning experiences and incorporate new knowledge into practice.
  • Maintain flexibility and adaptability to expected and unexpected changes in the work environment.
  • Report personal symptoms of suspected illness of contagious diseases to the Clinical Director.
  • Communicate effectively both orally and in writing.
  • Independently solve problems and follow through.
  • Organize and function independently in an office environment.
  • Perform other tasks and duties as assigned.
  • Complete all competency assessments as required by the facility.
  • Maintain certifications up to date including but not limited to: (CPR, Verbal De-Escalation, HIPAA, HIV/AIDS, Infection Control).
