About The Position
Coordinates service needs and collaborates with other services and agencies. Completes assessments and determines the appropriate level of services for new members served. Telecare's mission is to deliver excellent and effective behavioral health services that engage individuals in recovering their health, hopes, and dreams. San Mateo Transitions is an Assertive Community Treatment program providing services to 180 individuals. Special populations: 40 also receive residential service, 50 are older adults, and 55 are criminal justice clients.
Requirements
- Master’s degree in Social Sciences
- One (1) year of experience in mental health services
- Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
- Willingness to use your personal vehicle to drive clients to appointments and groups, etc. (weekly mileage reimbursement at the IRS rate)
- Willingness to use the company vehicle to drive clients to appointments and groups, etc.
- Willingness to use your personal vehicle to attend meetings, etc. (weekly mileage reimbursement at the IRS rate)
