About The Position

Join Our Team as a Case Manager! Are you passionate about Human Resources and making a real difference in the nonprofit sector? EAC Network, a leading not-for-profit social service agency dedicated to helping over 79,000 people through 100 impactful programs across Long Island and NYC, is looking for a part time Case Manager to join the Respite Companion program. Salary: $17-22/ hour Schedule: Monday-Friday, 9AM-5PM Location: Garden City, NY Reports to: Program Director What drives the core purpose of this job: The Case Manager is responsible for providing case management support for caregivers who care for an older relative and assists with the day-to-day operations of the In-home Respite Services program. In addition, the Case Manager supports outreach, caregiver assessment, data collection, and reporting activities.

Requirements

  • Associates Degree required, Bachelor’s Degree in human services or related field preferred.
  • Knowledge of resources and entitlements for senior citizens and prior experience working with the elderly preferred.
  • Must have excellent communication skills, both oral and written
  • Must be proficient in computer applications or other automated systems such as Microsoft Office Applications, email, and data base software necessary to preform work assignments.
  • Must have valid New York State Driver’s License and have access to an automobile.

Responsibilities

  • Maintain a case load of clients and monitor their participation in the program.
  • Maintain case documentation and client records.
  • Conduct initial home visits and re-assessments to monitor the provision of in-home Respite
  • Make referrals for entitlements and services to assist caregivers of older adults.
  • Assist with outreach to recruit companions and prospective clients.
  • Assist with screening and training of Respite Companions
  • Record telephone intakes and provide guidance and information for caregivers.
  • Process monthly fee collection records and review and submit time sheets.
  • Participate in quarterly NYSOFA/NYSCRC calls and other grant-related trainings and meetings.
  • Coordinate and facilitate caregiver awareness activities, including participation in and facilitation of caregiver simulations.
  • Conduct outreach activities to promote services and the availability of respite care to family caregivers in Nassau County, including coordination with NY Connects and the Nassau County Long Term Care Council.
  • Develop individualized caregiver support plans based on CII assessment results and available respite resources.
  • Complete required training and administer the ARCHANGELS Caregiver Intensity Index (CII) to assess caregiver need at intake and at required follow-up intervals.
