Under the direction of a case management supervisor, the position provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives. The role involves obtaining in-depth social histories, developing and assuring community and/or agency resources to meet the consumer's needs, and serving as a Qualified Intellectual Disabilities Professional (QIDP) while providing monthly monitoring of the program. This position is required to meet compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines.
