Rock Valley Community Programs provides transitional and supportive housing, addiction and mental health treatment services to adults in the Rock County area. We provide these services to homeless male veterans, clients connected to the criminal justice system, and outpatient care to anyone needing assistance with addiction and/or mental health treatment. The Residential SUD/DD Case Manager provides comprehensive case management, treatment coordination, and evidence-based group facilitation services to clients participating in a residential Substance Use Disorder (SUD) and Dual Diagnosis treatment program. This position supports clients who are working to address substance use disorders, mental health conditions, criminogenic risk factors, and barriers impacting successful community reintegration. The Case Manager is responsible for managing an assigned caseload of approximately 8–12 clients while facilitating approximately 10–12 hours of structured group treatment programming per week. This position requires the ability to build therapeutic relationships, apply evidence-based interventions, reinforce pro-social behaviors, challenge maladaptive thinking patterns, and support clients in developing skills necessary for long-term recovery and reduced recidivism.
