In alignment with WoodGreen’s mission to promote wellness, reduce poverty, and build sustainable communities, this role delivers essential services that foster self-sufficiency and housing stability. All work is carried out in accordance with WoodGreen’s policies, Ministry of Health standards, and all relevant provincial legislation to ensure the highest quality of care for our tenants. Reporting to the Supportive Housing Supervisor, the Case Manager provides specialized support to tenants within our Supportive Housing Programs who are living in bachelor-style units. These individuals often navigate prolonged mental health and addiction challenges or have experienced chronic homelessness. This position is vital to maintaining affordable and sustainable housing, with a primary focus on providing specialized interventions for hoarding and pest control management. Working as part of a multidisciplinary team of Client Support Workers and Case Managers, you will provide comprehensive clinical support, including client assessments, case planning, and crisis intervention. You will act as a key liaison between our clients and the broader community by making referrals to appropriate resources and collaborating closely with healthcare providers and agency partners to ensure a holistic approach to client wellness.
