About The Position

The Community Service Board of Middle Georgia's Emerging Adult program is seeking a qualified individual to serve as a PSR-I A Case Manager. This role adheres to treatment plans and provides services and interventions as planned. The Case Manager is responsible for recognizing consumer needs and assisting them in obtaining appropriate services. They demonstrate consideration for others, work cooperatively with co-workers, provide constructive feedback, and appreciate differences in approaches, personalities, and viewpoints. The Case Manager will work with all involved to ensure the team moves in the same direction, everyone stays informed, and monitors needs and progress.

Requirements

  • High School Diploma/GED Equivalent
  • Valid Georgia Driver’s License
  • Effective verbal and written communication skills
  • Strong interpersonal skills and the ability to work effectively with diverse communities
  • Ability to work independently and in collaboration with others
  • Experience with Microsoft 365 Office Products
  • Ability to organize, prioritize and meet deadlines accordingly

Nice To Haves

  • Bachelors Degree in a related field (Social Work, Psychology, Human Services, Etc.)
  • Previous experience with case management or behavioral health

Responsibilities

  • Make required contacts per month to consumers per standards for PSR-I/CSI (minimum of 2 per month, though daily contact may be needed for this population).
  • Adhere to treatment plans and provide services and interventions as planned.
  • Write progress notes (within 24-48 hours) and bill for services provided.
  • Adhere to DBHDD guidelines regarding the quality of documentation.
  • Complete notes within the agency policy timeline.
  • Recognize consumer needs and assist them in obtaining appropriate services.
  • Provide consumer information for outside resources and encourage them to access them as independently as possible.
  • Communicate accurate information to others in a professional and courteous manner; convey a willingness to assist.
  • Show consideration for others, work cooperatively with any co-worker, provide constructive feedback without undue criticism of others; display appreciation of differences in approaches, personalities, and viewpoints of others.
  • Solicit input from those affected by plans or actions; give credit and recognition to others who have contributed; demonstrate concern for treating people fairly and equitably.
  • Accept responsibility for own mistakes and take action to prevent similar occurrences; work to resolve conflicts and identify solutions in which all parties benefit.
  • Identify team goals and ways to work with coworkers to accomplish those goals; work to keep group activities productive/focused on results.
  • Demonstrate eagerness to learn and assume responsibility; seek out and accept increased responsibility; display a "can do" approach to work.
  • Show persistence and seek alternatives when obstacles arise; seek alternative solutions; do things before being asked or forced to by events.
  • Work within the system in a resourceful manner to accomplish reasonable work goals; show flexibility in response to process changes and adapt to and accommodate new methods and procedures.
  • Accept direction and feedback from supervisors and follow through appropriately.
  • Attend in-service training as required.
  • Attend monthly staff meetings.
  • Attend meetings with other agencies/court as requested.
  • Contact Supervisor/Clinical Supervisor to obtain guidance and assistance when necessary.
  • Review available professional literature and educational material in a timely manner to improve the level of knowledge/skills.
  • Complete all duties assigned in a timely manner.
