About The Position

Our client, a faith-based nonprofit supporting individuals transitioning from incarceration, is seeking a Case Manager to provide ongoing support to program participants as they re-enter society. This role focuses on building meaningful relationships, assessing individual needs, and developing personalized plans of care to guide each client’s success. The Case Manager partners with community organizations, connects clients to critical resources, monitors progress, and maintains consistent engagement through one-on-one interactions, all while documenting services and outcomes to support long-term impact.

Requirements

  • Provide ongoing support to program participants as they re-enter society.
  • Build meaningful relationships with clients.
  • Assess individual needs and develop personalized plans of care.
  • Partner with community organizations to connect clients to critical resources.
  • Monitor client progress.
  • Maintain consistent engagement through one-on-one interactions.
  • Document services and outcomes.
  • Plan and organize collaborative services.
  • Develop and administer a Plan of Care for each client.
  • Achieve consistent interactions with clients, including service distribution, phone calls, and one-to-one sessions for up to a year.
  • Partner with Florida Department of Corrections (FDC) to recruit potential clients from correctional facilities, transitional housing facilities, Halfway Houses, and Work Release Facilities.
  • Schedule events or attend community events to raise program awareness and gauge community interest.
  • Visit FDC supervision offices and local re-entry correctional institutes weekly to present the organization's inspHire Program and collect information on potential clients.
  • Use a variety of techniques to determine clients’ needs.
  • Complete individual Plan of Care assessments and interviews.
  • Make independent professional recommendations based on the outcomes of implemented and monitored treatment plans.
  • Maintain or exceed the organization's established standards for program performance relative to each client’s growth and development through program participation.
  • Resourcefully manage the client’s professional and personal success.
  • Communicate, connect with, and collaborate with community agencies to offer program clients a wide range of resources.
  • Interpret available resources and skillfully convey information to clients.
  • Follow up with clients and agencies as appropriate to document use/referral success.
  • Meticulously enter all client interactions and transactions into the organization's Efforts-to-Outcomes (ETO) data system.
  • Perform any other tasks assigned by inspHire leadership team.
