About The Position

Greystone Foster Care, in partnership with Universal Health Services, Inc., is seeking a dedicated and compassionate Case Manager to join their growing team. This role is crucial in providing comprehensive behavioral health care services to at-risk youth struggling with extensive mental health issues. The Case Manager will work with therapeutic foster parents who provide a safe, stable, and therapeutic environment for children and youth removed from their homes, linking them to hope, healing, and trust. This is a hybrid position, blending remote work with in-office requirements and occasional facility reporting. The role demands full-time dedication and flexibility to work evenings and weekends.

Requirements

  • Bachelor's degree in Social Work or related field.
  • One to three years' related experience and/or training, or equivalent combination of education and experience.
  • Valid state Driver's License.
  • Automobile insurance with verification of medical liability.
  • Must reside in the Northwest/Middle TN area of the state: Montgomery, Cheatham, Stewart, Robertson, Sumner, Davidson, and Dickson counties.
  • Requires a successful background check, fingerprinting and employment verification.

  • Master's degree preferred.

Responsibilities

  • Complies with state regulatory requirements and Greystone Foster Care plan.
  • Ensures that all placement documentation is completed according to established timelines.
  • Completes regular file audits for each child and participates in regular peer file audits to ensure audit preparedness at all times.
  • Assists with completing foster parent reassessments when necessary.
  • Schedules child/family team meetings at least quarterly and corresponding documentation is completed/submitted.
  • Visits each assigned child per policy or as needed.
  • Ensures that annual EPSDT and biannual dental appointments are scheduled and information provided to DCS.
  • Ensures that outpatient mental health assessments are scheduled for all Level 2 youth and participation in recommended services occurs.
  • Obtains required consents and releases from appointed guardians for medical appointments and medications.
