About The Position

This role involves providing comprehensive case management services to individuals with behavioral health needs. The Case Manager will be responsible for conducting assessments, developing goal plans, overseeing admissions and discharges, providing individualized support, and participating in treatment team meetings. The position requires a person-centered approach, strong critical thinking, crisis intervention skills, and the ability to advocate for individuals from diverse backgrounds. The Case Manager will also play a key role in managing residential responsibilities, ensuring a positive and therapeutic environment, and maintaining health and safety standards.

Requirements

  • Associates or Bachelor’s degree in Human Services, Psychology, Counseling or related field and/or two years’ experience in working with behavioral health consumers
  • Experienced in providing trauma informed care
  • Ability to work in cooperation with internal and external personnel and agencies
  • Ability to work with and advocate for individuals with diverse backgrounds
  • Person-centered approach and strong critical thinking skills
  • Strong crisis prevention and intervention skills
  • Certified in First Aid, CPR, and as a Medication Aide before completion of probationary period
  • Valid Nebraska Driver’s License and insurable with a good driving record
  • Strong organizational, interpersonal, written and verbal communication skills
  • Ability to perform efficiently and accurately in a fast-paced, deadline-oriented work environment
  • Ability to multi-task when necessary
  • Exemplary stress management skills
  • Non-judgmental, caring attitude toward persons served
  • Strong computer skills
  • Familiarity with CARF standards and a willingness to conform to all standards
  • Ability to work full-time (a minimum of 40 hours a week) to include evenings and/or weekends as required

Responsibilities

  • Conduct Orientation activities, including needs assessments and developing case management goal plans.
  • Oversee Respite admission, ongoing daily activities, and coordinate discharge.
  • Provide individualized case management services, including weekly meetings with residents, documenting activities, and providing referrals.
  • Actively participate in weekly case staffing/treatment team meetings, tracking and reporting case management activities, and identifying progress and barriers.
  • Manage the Level System for residential consumers, including orientation, preparing level sheets, communicating progress, and assisting residents in navigating requirements.
  • Ensure the overall environment remains positive, trauma-informed, and therapeutic by being present with residents, role-modeling activities, assisting with problem-solving, and employing various intervention techniques.
  • Maintain the health and safety of the environment by being aware of resident whereabouts, enforcing program rules, keeping the facility clean, conducting safety drills, and supervising residents in household tasks.
  • Participate in treatment activities of the residents, including transporting residents, developing and facilitating educational groups, and completing Medication Aide responsibilities.
  • Conduct transition and discharge activities, beginning discharge planning from admission and working towards a strong discharge plan.
