About The Position

This is a full-time Case Manager position at Youth Housing Opportunities (YHO) in Cincinnati, OH. The role involves connecting youth and families with resources, supporting their goals, and acting as a champion for them. It is not a desk job and involves varied daily activities such as helping teens connect with resources for school, celebrating family milestones, coordinating services, and assisting individuals in navigating difficult situations. The core mission is to help youth and families build brighter futures by connecting them to resources, supporting their goals, and ensuring they have a supportive team.

Requirements

Bachelor's degree in Human Services or related field OR High School Diploma/Associate Degree plus 3 years of relevant experience
QMHS certification
Reliable transportation and willingness to travel throughout the region

CDCA certification

Responsibilities

Become a trusted ally for youth and families.
Help clients identify goals and create a roadmap to achieve them.
Connect people with resources.
Partner with schools, providers, community agencies, and support systems.
Support clients through challenges, setbacks, and victories.
Help youth build skills for independence, education, employment, and everyday success.
Coordinate care so everyone stays focused on the client.
Manage crises with calm, confidence, and compassion.
Create service plans, document progress, and keep everything moving forward.
Celebrate wins—big and small.
