About The Position

Under general supervision, provide case management, specialized supportive, supplemental services to clients to aid them in improving their social, economic, cultural or educational status or to assist them in restoring their capacity for social functioning. This program serves youth and it is a prevention and supplemental behavioral health program that collaborates with a multitude of community resources with the aim of providing stability and improving functioning of family members. Services also provide the caregivers opportunities to enhance their parenting strategies. This is a grant funded position until June 2027.

Requirements

  • Combination of education and experience normally represented by a Bachelor's Degree and one to two years related professional experience.
  • Must have competent oral, written and interpersonal communication skills.
  • Must be well organized, detail oriented and have the ability to maintain confidentiality.
  • Ability to work with diverse client population, community organizations as well as staff at all levels.
  • Must be able to form good working and therapeutic relationships.
  • Final applicant is required to be fingerprinted to complete background check.

Nice To Haves

  • Bilingual in Spanish preferred.

Responsibilities

  • Under direction, ensure that the activities of this position and relevant programs are consistent with the mission, vision, and values of Catholic Charities, Diocese of Cleveland.
  • Work with children, caregivers, and parents to address unmet needs and support them as they navigate through their barriers.
  • Provide direct service either in the office, via telehealth or in the field/homes.
  • Initiate and coordinate interventions, review and monitor progress, and reassess needs to ensure successful outcomes.
  • Assist clients in developing formal and informal community supports, while partnering with local agencies and resources.
  • Provide advocacy on behalf of clients.
  • Prepare and complete all required case recordings in an electronic medical record, administrative reports, statistical reports and related paperwork or database records in a timely and professional manner.
  • Attend clinical and administrative staff meetings, and multi-discipline team meetings as required.
  • Review treatment plans and progress of cases with supervisor for continued support, advice and approval.
  • Perform other duties as necessary to accomplish objectives.
