About The Position

Mary T Maryland is hiring for a full-time Case Manager to be based out of our main office located in Millersville, MD. The Case Manager will work under the direction of the Program Manager to ensure the proper communication delivery, education and follow-up of med delivery for consumers at their assigned homes and programs of Mary T Maryland. The ideal candidate will possess excellent communication skills, organizational skills, and attention to detail.

Requirements

  • High School Diploma/GED required
  • Valid Driver’s License
  • 2+ years of experience in med delivery and case management preferred
  • Cooperative and professional ability to assist others
  • Well organized and detail oriented
  • Ability to perform under pressure
  • Creativity and ability to implement creative activities with clients and staff
  • Strong interpersonal, verbal and written communications skills
  • Ability to multitask
  • Strong computer and software skills

Nice To Haves

  • LPN or Medical Assistant certification preferred

Responsibilities

  • Communicate, educate, document, deliver and follow – up on medication related activities for consumers at the homes and programs for Mary T Inc, Maryland
  • Attend weekly and monthly meetings with treatment teams and management.
  • Facilitate, coordinate, and attend all consumer physician appointments and other medical appointments.
  • Responsible for preparing clients and paperwork for scheduled medical appointments in a timely manner.
  • Manage and monitor medication changes.
  • Conduct monthly quality assurance checks on medications books and daily logs in assigned locations.
  • Facilitate effective communication regarding client’s health and medications to staff in various programs and effective with physicians.
  • Facilitate effective communication with family members of clients and provide status updates.
  • Facilitate client's medical equipment repairs.
  • Filing of all appointments and medication information immediately following appointments.
  • Handling client medical insurance issues, preauthorization for specialty medical appointments.
  • Filing applications and redeterminations of all medical assistance for clients.
  • Ordering all medical supplies for the residential homes.
  • Consistent communication with the entire team on medical updates for clients
  • Review incident reports for medical follow up as needed
  • Will be present at admissions to ensure a smooth transition for the client
  • Assist with being the point of contact for any hospital admissions and ensure that all discharged information is given to the facility and update DHCA and team as needed with client status
  • Accompanying the nurse to the residential homes during the 45-day nursing visit.
  • Complete 45-day nursing report plans of correction
  • Develop and complete initial and annual behavior modifying medication plans for presentation at the Human Rights Committee.
  • Assist clients with Activities of Daily Living and direct care as needed
  • Assist with medication training for staff at assigned locations
  • Participate in on-call rotation.
  • Assist management in achieving Mary T Inc quality and standards of excellence goals
  • Other duties as assigned
