About The Position

We are seeking a skilled Registered Nurse Case Manager to assess patient needs, coordinate care, and ensure effective communication among all parties involved in patient care. The role oversees coordination of nursing care, home health aide services, and applicable therapies after approval by the Department of Labor. It also requires maintaining confidentiality of client and office records in accordance with HIPAA guidelines and the Privacy Act PHI, and supporting cohesive, comprehensive care through communication and collaboration among the care team.

Requirements

• Hold a valid and active Registered Nurse license in good standing.
• Have at least one year of experience in a home health setting or related area, with strong assessment skills.
• Have excellent organizational and time management skills to handle multiple clients efficiently.
• Be proficient in computer and internet-based applications, as well as office equipment.
• Have competent working knowledge of client-based equipment.
• Pass a criminal background check and sanction screening.
• Maintain a professional demeanor and appearance.

Responsibilities
