About The Position

That’s why our teams are comprised of compassionate, dedicated professionals from a variety of backgrounds who collaborate to provide the very best evidence-based care for our clients at all levels of care. Case Managers work in collaboration with the clinical team to identify, coordinate, and secure community-based resources to support the Adolescent client’s ongoing treatment and recovery. The case manager also facilitates the admission and discharge process, coordinates outpatient care, and maintains up-to-date knowledge of relevant community resources and the procedures for accessing services from external providers and social agencies.

Requirements

  • Bachelor’s degree in mental health related field required
  • Must have a valid driver’s license and be authorized to drive

Nice To Haves

  • Licensed Social Worker preferred
  • Mental health case management experience preferred

Responsibilities

  • Facilitates the admission process by partnering with the admissions team and site staff to arrange for client needs
  • Assesses post discharge needs and communicates client discharge needs to the treatment team. May partner with families or other supports to coordinate discharge plans
  • Partners with Accanto staff to coordinate internal steps to lower levels of care and with external step-down providers and referring providers to coordinate client care including treatment appointments, lodging, and other community resources
  • Documents in the electronic health record in accordance with Accanto policies and other applicable regulatory standards
  • Maintains working knowledge of community resources and referral procedures. Networks with other organizations to foster referral relationships
