Lutheran Social Services of Southern California (LSSSC) is a non-profit social service agency founded in 1946, dedicated to serving individuals and families in need throughout Southern California. LSSSC's mission is to live out God's love by embracing, equipping, and empowering vulnerable individuals, families, and communities toward self-sufficiency. The Case Manager role is crucial in coordinating community-based services tailored to individual needs, focusing on mental health, medical health, and human services. This position is responsible for assisting individuals and families experiencing homelessness in securing housing, including interim housing, transitional housing, and finding permanent solutions through programs like Rapid Re-Housing (RRH). For RRH, the Case Manager will work with low-income individuals and families to prevent or end homelessness by providing financial assistance through Homeless Housing Assistance Prevention (HHAP) services. Key responsibilities include eliminating barriers to housing stability such as employment, income increase, financial literacy, budgeting, addressing health needs, and connecting clients to resources for transportation, childcare, and legal aid. The role utilizes strength-based intensive case management, advocacy, landlord mediation, and outreach to foster self-sufficiency and community engagement. Case Managers manage multiple cases, build on participant strengths, develop person-centered service plans, facilitate groups, and connect participants with necessary resources. Creative problem-solving, meticulous record-keeping, and strong community/landlord relationships are essential.
