Unison Health is seeking Case Managers to support children and adolescents with behavioral health needs in building skills for healthy development and daily functioning. Responsibilities include completing assessments, facilitating age-appropriate individual and group sessions, monitoring progress, providing crisis support, engaging families/caregivers, connecting clients to community and school-based resources, and helping youth develop coping, social, and life skills.

Requirements

  • Requires a bachelor’s degree in social work, psychology, recreation therapy or art therapy or related field or high school diploma or equivalent with at least three years of experience.
  • Required CPR certification within 90 days of hire.
  • Required CPI certification within 90 days of hire.
  • Must be at least 21 years of age.
  • Must demonstrate knowledge of mental illness including the SED population, crisis management, and the ability to establish and maintain therapeutic relationships with clients.
  • Must be able to work a flexible schedule that may include evenings, weekends and holidays.
  • Must demonstrate the ability to define problems, analyze data, establish facts and draw valid conclusions and make recommendations.
  • Must demonstrate a positive attitude, commitment to provide superior service, and enthusiastic support for the mission of the agency.
  • Must establish and maintain effective working relationships.
  • Must demonstrate the ability to represent the agency in a positive manner when in the community.
  • Must be able to work with ill, disabled, emotionally upset and, at times, hostile individuals.

Responsibilities

  • Deliver therapeutic behavioral services, including assessments, treatment planning, service coordination, progress monitoring, crisis intervention, and client advocacy.
  • Provide transportation for clients, ensuring safe, timely travel and communicating any concerns.
  • Maintain agency vehicles by inspecting, cleaning, fueling, and reporting maintenance issues.
  • Document services and maintain accurate records in the electronic health record per policy.
  • Ensure client confidentiality and proper handling of information in compliance with regulations.
  • Assist with medication administration under nurse supervision and provide services within licensure and approval.
  • Follow agency policies and procedures while supporting unit operations and additional assigned duties.
