Requirements

Bachelor's degree in Human Services or a related field required, preferably: Social Work, Psychology, Rehabilitation Counseling, Sociology, Counseling, Or another closely related discipline
Knowledge of behavioral health theory, evidence-based practices, and emerging treatment approaches is preferred.
Previous case management experience in behavioral health, social services, child welfare, or a related field is preferred.
Experience working with children, adolescents, and families is highly desirable.
Strong relationship-building and communication skills.
Compassion, empathy, and professionalism when working with youth experiencing behavioral health challenges.
The ability to collaborate with multidisciplinary teams and community partners.
Excellent organizational, documentation, and time-management skills.
Sound critical thinking and decision-making abilities in complex or changing situations.
Knowledge of case management principles and behavioral health services.
Understanding of applicable laws, regulations, and community resources related to youth and family services.
Ability to manage multiple priorities while maintaining quality and accuracy.
Cultural competence and commitment to serving diverse populations.
Emotional resilience and the ability to remain calm and professional during crisis situations.
Physical and emotional ability to safely support clients who may exhibit challenging verbal or physical behaviors.
Proficiency using computers, electronic health records, and other technology required for the position.

Responsibilities

Build trusting, supportive relationships with youth and their families while engaging them as active partners in their care.
Conduct comprehensive assessments and contribute to individualized treatment and care plans.
Coordinate services with behavioral health providers, medical professionals, schools, social service agencies, and community partners.
Advocate for youth and families by helping them access resources, benefits, and community supports.
Serve as a liaison between clients, families, and service providers to ensure coordinated, high-quality care.
Support youth and families through crisis situations by helping identify solutions and coordinating appropriate interventions.
Exercise sound professional judgment when evaluating complex situations and determining appropriate courses of action.
Promote client strengths, resilience, and independence while encouraging positive growth and recovery.
Develop and maintain strong partnerships with community organizations and referral sources.
Represent clients effectively when working with courts, educational systems, healthcare providers, and social service agencies.
Demonstrate cultural awareness, respect, and sensitivity in every interaction.
Complete timely, accurate documentation that meets organizational, state, and federal requirements.
Participate actively in supervision, team meetings, professional development, and ongoing training.
Travel throughout the service area to meet with youth and families in homes, schools, community settings, and other locations as needed.
Provide direct support and transportation for clients and families when appropriate.
Stay current on best practices, evidence-based interventions, and applicable regulations, compliance requirements, and ethical standards.
Perform other duties as assigned to support quality client care.
