The Case Manager Youth Engagement is responsible for providing pre- and post-employment case management services, social services navigation, coaching, and other supportive services to youth ages 16 – 24 within Metro Chicago. The Case Manager provides support and resources to individuals in need of support to the underserved population and address barriers to employment to include transportation, soft and hard skills development, access to critical resources such as housing and healthcare, and other social, cultural, and environmental conditions that impact the individual’s life. The Case Manager will help to identify areas of need and work with the individual to identify goals and develop plans to address these barriers in an effort to allow the individual to prepare for, secure and maintain employment. Implements strategies to achieve the goals for the organization and Mission Services. Implements, revises, and sustains policies, procedures and programs. Typically works on projects and initiatives that span 3 – 12 months.
