About The Position
This position is for a Case Manager whose responsibilities includes assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating inter-disciplinary care activities; assists with Swing Bed Program: and coordinates the continued care of transitioned patients.
Requirements
- Education level equivalent to completion of an associate degree in nursing
- Current license to practice professional nursing in the state of Georgia
- BLS Provider certification required
- Basic technical nursing skills
- Experience in chart auditing
- Experience in interpreting federal and state regulations and accreditation standards governing patient medical necessity and continuity
- Basic typing and computer skills
