About The Position

Rosecrance is seeking a Case Manager for its Therapeutic Intervention Program (TIP) Court. This role provides mental health rehabilitation services and support to adults and families, aiming to reduce hospitalizations and crisis episodes while enhancing community functioning. The goal is to help clients achieve rehabilitative, resiliency, and recovery objectives. The Case Manager will ensure program services are delivered ethically, legally, and morally within a safe and therapeutic environment, adhering to regulatory and accreditation standards. This position is a team-oriented role focused on delivering mental health services to clients and their families, integrating recovery principles into daily life. Specific duties include case management, crisis intervention, skills training, timely documentation, individual counseling, medication monitoring, attending meetings, and quality improvement initiatives to meet the needs of individuals with serious mental illness.

Requirements

  • Bachelor’s Degree with minimum two years’ experience in mental health.
  • Demonstrated education, training, or experience in mental health services.
  • MHP designation.
  • Valid driver’s license.
  • Adequate written skills to accurately complete required documentation within the time frames prescribed.
  • Excellent organizational and computer skills.
  • Ability to work independently and with a team.
  • Skilled in conducting individual, family, and group counseling, and case management.
  • Ability to present and coordinate lectures for clients, families, staff, or community resources.
  • Ability to motivate and problem solve with staff regarding effective clinical practices.
  • Ability to be self-motivated, seek improved performance, and problem-solve while meeting complex client and family needs.
  • Skills to adapt verbal and written communication to the language and style of the client and their family.
  • Complete all necessary documentation within the appropriate time lines.
  • Maintain communications with all staff (e.g., admissions, process, and discharge plans).
  • Valid driver’s license and ability to meet Agency insurance requirements.
  • Must have own transportation.

Responsibilities

  • Provide mental health rehabilitation services and supports to adults and families.
  • Decrease hospitalization and crisis episodes.

  • Coordinate services from a variety of community resources.
  • Utilize a variety of treatment concepts, evidence-based practices, and service modalities.
  • Present workshops, lectures, and training to clients, families, staff, and community groups.
  • Be familiar with court proceedings and available for court appearances on behalf of Rosecrance clients.
  • Serve as a member of the Clinical Team and participate in all team meetings and activities.
  • Exercise confidentiality in keeping with the professional Code of Conduct and within the framework of the law.
  • Deliver exceptional customer service consistently.
  • Serve as a role model and demonstrate positive guest relations.
