About The Position

Hamilton-Madison House’s Supported Housing program provides long-term housing options for individuals with Serious and Persistent Mental Illness (SPMI), enabling them to live independently within the community. The Case Manager supports tenants in maintaining stable housing, improving independent living skills, and accessing community-based resources. This role emphasizes recovery-oriented, trauma-informed care and adherence to OMH guidelines and standards.

Requirements

  • Bachelor’s degree in Social Work, Counseling/ Psychology or related field required
  • At least 1 years of experience working with individuals with serious mental illness or in supportive housing
  • Knowledge of trauma-informed care principles
  • Strong organizational, documentation, and communication skills
  • Ability to work both independently and as part of a multidisciplinary team

Nice To Haves

  • Bilingual proficiency in English and Korean preferred
  • Familiarity with OMH regulations and supportive housing models preferred

Responsibilities

  • Provide ongoing case management services to a caseload of tenants in supportive housing
  • Conduct initial and ongoing assessments of tenant needs, strengths, and goals
  • Develop, implement, and regularly update individualized service plans in collaboration with tenants
  • Assist tenants in obtaining and maintaining benefits (e.g., SSI/SSD, SNAP)
  • Support linkage to mental health, medical, and substance use treatment providers
  • Conduct regular home visits to monitor housing stability, safety, and well-being
  • Provide crisis intervention and de-escalation as needed
  • Advocate on behalf of tenants with landlords, service providers, and community agencies
  • Maintain accurate, timely, and compliant documentation in accordance with OMH and agency standards
  • Participate in team meetings, case conferences, and supervision
  • Facilitate life skills training, including budgeting, and daily living skills
  • Promote community integration and social engagement
