About The Position

Chartiers Center is a private, non-profit corporation funded by state, county, third party insurances and private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day, and recreational services for adults with intellectual disabilities, homeless outreach, and housing program. As a Case Manager Supervisor with Chartiers Center, you will be responsible for the daily management of the Case Management Team and the coordination of activities to ensure that services are delivered.

Requirements

  • Master’s degree in human services or related field and 3 years mental health direct experience OR Bachelor's degree in human services or related field and 5 years of mental health direct experience
  • Valid Driver’s License Required

Responsibilities

  • Overseeing the implementation of Core Affirmative SPA Responsibilities by staff to ensure timely and effective provision of resources to consumers; clear communication regarding expectations; periodic assessment and cross system planning to meet consumer needs based on strengths and desire.
  • Ensuring continuity of care through monitoring caseloads to ensure standards are met; monitor timely submission of authorizations for care; process referrals in an accurate and timely manner.
  • Reviewing and approving of all Service Plans and related documentation; and collaborate with internal and external program staff.
  • Providing guidance and clinical support to staff through individual and group supervision to ensure timely and quality of care.
  • Providing field supervision and support in the development of competencies to staff.
  • Completing necessary outreach and screening activities to individuals/natural supports considering participation in the program.
  • Serving as a member of a multi-disciplinary and multi-agency team and as such, work in a cooperative and collaborative manner.
  • Developing and managing the on-call system for Case Managers.
