About The Position

Carolina Family Health Centers, Inc. is seeking a Population Health Manager (Registered Nurse) - Medicaid Managed Care to oversee the day-to-day activities of the Population Health Program. This role involves direct supervision of case managers and care coordinators who provide care management, transition of care, care coordination, and other population management activities to CFHC patients, with a focus on those with Medicaid. The manager will be knowledgeable in case management principles, including Medicaid managed care, and will work closely with the CFHC, Inc.’s clinically integrated network. The position also includes providing case management services to a small caseload of patients. This position offers a 40-hour workweek, no weekends, a full benefits package, 12 paid holidays, a bi-annual bonus, and bi-weekly pay.

Requirements

  • Graduate of an accredited school of nursing.
  • Licensed as a Registered Nurse in the State of North Carolina.
  • Three years in a healthcare environment with supervisory/management background and experience in case management.
  • Must have personal vehicle readily available to freely travel in the local area; between clinics, hospitals, health departments, etc.
  • Active NC Driver License.
  • Registered Nurse (RN).

Responsibilities

  • Provides direct supervision to the case managers and care coordinators, including interviewing, hiring, providing training and orientation, distribution and determination of responsibilities, and ongoing performance evaluation.
  • Monitors staff productivity and department success through regular reporting and communication to executive staff.
  • Performs audits and runs reports to monitor staff adherence to CFHC, Inc., the Carolina Medical Home Network, and other regulatory agencies' rules and regulations for providing and documenting case management services.
  • Represents CFHC, Inc. at Carolina Medical Home Network meetings.
  • Attends other meetings and participates in committees as assigned (e.g., Population Health CIT).
  • Reviews software system functionality and develops and documents efficient workflows for staff.
  • Serves as a point of contact for local hospitals’ discharge planners to identify patients admitted to the hospital or those who have used the emergency room, aiming to reduce 30-day hospital readmission rates or inappropriate emergency room use.
