Telecare's FPATH #269 is seeking a Washington State Substance Use Disorder Professional (SUDP) who wants to do meaningful, hands-on work with adults living with serious mental illness and co-occurring substance use challenges. This is not a traditional office-based counseling position. You will work directly with members in the community, helping them address substance use, behavioral health needs, housing, healthcare, daily living needs, and other barriers that can impact long-term stability. Using a harm-reduction, recovery-focused approach, you will meet people where they are, build relationships over time, and help members identify and work toward recovery goals that are meaningful to them. The program is grounded in the recovery model and emphasizes: Harm reduction and long-term recovery, Person-centered and individualized care, Community-based engagement, Housing and community stability, Building independence and resilience, Respect for each member's goals, choices, and stage of recovery.
