This role recommends and assists clients in accessing medical, educational, social, vocational, housing, and other services to support program goals. The Case Manager initiates referrals to next level care, coordinates with physical health providers for whole-person care, and advocates in the best interest of clients.

Requirements

  • High School Diploma or equivalent required; active SUD registration required at minimum.
  • Valid CA driver’s license and current auto insurance per contract requirements required.
  • Cultural competency understanding required.
  • Per program contact requirements, must satisfactorily complete a Tuberculosis test prior to hire and once annually thereafter.

Nice To Haves

  • Experience providing mental health and/or substance use disorder services preferred.
  • Bilingual preferred.
  • Knowledge of Microsoft Office and EHR software preferred.
  • CPR/First Aid certification (preferred), and bi-annual thereafter.

Responsibilities

  • Provide case management with crisis intervention, discharge plan, group & individual work, mental health/substance use disorder counseling, screening, and assist clients in meeting basic needs and symptom management.
  • Complete accurate and timely documentation, meet minimum billable requirements for direct client services if contractually required.
  • Facilitate placement including pre-placement visits, and transportation as needed.
  • Shift coverage including part of the on-call team.
  • Provide field-based services as required by program and client need.
  • Maintain confidentiality of client’s care.
  • Participate in scheduled program meetings.
  • Prepares and maintains client’s case management goals and facilitates monthly housing meetings.
  • Maintains daily log of client contacts.
  • Records data input of client’s demographic information.
  • Works closely with program direct service staff to coordinate appropriate housing options for clients relative to functioning level and individual needs and preferences.
  • Provides outreach in the community and agencies and attends meetings to provide program education.
  • Other relevant duties as assigned.
