The job involves implementing and conducting case management, services, and training for participants in specialized Mission Services programs. The role requires conducting case management functions such as program applicant screenings, assessments, enrollments, documentation collection and follow-up activities. It also involves providing support services to enhance program participants successes, creating Individualized Career and Financial Plans (ICFP), conducting job readiness and life-skills trainings, administering program evaluations, and providing coaching opportunities. The Case Manager - Success Coach must maintain an encouraging, positive, and respectful learning environment, meet regularly with participants, input all participant documentation into the tracking database, and ensure participant files are adequately maintained and confidential. Additionally, they will provide documented monthly participant successes/stories and ensure adequate materials, supplies, and equipment are available.
